I confirm that I am medically fit to participate in SparkAbility 2026: A Walk for Change and have assessed the accessibility level of the route for my own participation. I acknowledge that despite the organisers taking all reasonable safety measures, inherent risks remain in my participation and I accept full responsibility for my decision to take part. I will not hold the organisers responsible for any injury, loss or damage arising from my participation.

Parental/Guardian Consent

I, the parent/guardian of the participant, has read the rules and regulations of the event and allow the participant to sign up and take part in this event. I shall not hold the organiser, its officers, staff, volunteers, agents, contractors, event officials, event sponsors and event partners from all and any demands, claims, liabilities, proceedings, suits, losses and/or damages of any kind whatsoever arising out of or in connection with his/her participation in the event, including without limitation, personal injury and/or death.

PDPA Consent *

I consent to and allow the Organiser to collect, use, disclose and/or process my personal data in order to process, administer, facilitate, maintain and/or manage my relationship with the Organiser as a member, volunteer, programme participant, beneficiary and/or donor (“Purpose”), including communications on the Organiser’s activities, programmes and services; donation requests; carrying out research, analysis and development activities for the Organiser’s purposes; and making disclosures required by law or a competent authority.
